Register For Our Workshops

The following are required for enrollment in our cosmetic surgery workshop courses:

NOTE: Balance is due 4 weeks prior to the first day of the course.

Instructions for Surgeons

1. Complete and sign Surgeons Enrollment Application Form

2. Send copy of current Curriculum Vitae and liability insurance policy (malpractice coverage), as well as a copy of your active medical license.

3. Pay $1800 deposit with application.
Use credit card by phone or mail check payable to E. Antonio Mangubat, M.D.

Please write the title of the course in the memo area of the check!

Instructions for Nurses / Medical Assistants

1. Complete and sign Nurses/Medical Assistants Enrollment Application Form

2. Send copy of current Curriculum Vitae

3. Pay $250 deposit with application. Total fee $500.
